Ah, then your unzip tool did that for you. Not very clever of it. Fire 
it and find a new one.

It's OK to move the index.php, but you have to be extra careful to then 
set the options correctly. It certainly looks like you have it set 
correctly now. Sorry for snapping at you.

Definitely. It can't work unless that name is correct.
